We use your responses to generate your educational research profile and, where you have asked for it, to send that profile to you and follow up about a professional review request. We do not sell your information.

Storage

Your responses are held in your browser so your report can be displayed. If you submit contact details, they are transmitted to us for the purposes described above.

This website is not a healthcare provider and this assessment is not a HIPAA-covered transaction. Please do not submit detailed medical records, diagnoses, or clinical notes through this website.

Your choices

You can leave the assessment at any point without submitting contact information. You may request deletion of information you have submitted by contacting us.
